*Acknowledgment of Non-Liability: Before services rendered clients agree to participate in the activities provided by Mars Companion Care. The client understand that despite precautions taken by Mars Companion Care, there are inherent risks associated with the services provided working with Geriatrics and less stable medical outcomes. I hereby release Mars Companion Care, its employees, and affiliates from any and all liability for injuries, damages, or losses arising from my participation. I confirm that I am participating voluntarily and acknowledge that I have read and understood this statement before receiving services.
